表面上你的PHP和HTML看起来还不错。看起来你错过了你的内容，因为生成PDF的按钮没有通过$id变量。您至少应该将表单修改为以下内容：

此外，你实际上并没有对你的表单做任何事情（即没有表格字段可以通过），所以你也可以做一个链接。使用像Bootstrap这样的UI框架时更容易一些，因为您可以设置链接样式，使它们看起来像是按钮。

至于CSS，它实际上取决于你如何引用外部文件（如果它是外部的）。有必要查看您正在向Dompdf提供的HTML以调试问题的这一部分。
